Due to the COVID-19 pandemic, and for the protection of our patients, we ask that if you answer ‘Yes’ to any of the questions below and you have an appointment, please call our office to reschedule your visit. 1. Do you have fever, new or worsening cough, or flu-like symptoms?
2. Have you or someone you are in close contact with traveled out of the country in the last two weeks?
3. Have you been in close contact with someone confirmed to have the Coronavirus Disease 2019?
